PReS-FINAL-2198: Insufficient efficacy of tocilizumab therapy in children with Castleman's disease

چکیده
Introduction Castleman’s disease (CD) or angiolymphoid vascular hyperplasia is a rare and not well recognized disease of unknown etiology characterized by a massive growth of lymphoid tissue that was first described in 1954, and subsequently better defined by Castleman in 1956. It usually appears in young adults whilst it rarely occurs in childhood. There are only about 100 pediatric cases published. CD is believed to be autoinflammatory in pathology and result in increased interleukin-6 secretion. Previous studies in adult patients suggested beneficial role of the anti-interleukin-6 receptor antibody tocilizumab in treatment of CD. We report 3 cases of multicentric Castleman’s disease in children, treated in our center in recent years.

A 38-year-old woman was referred to our hospital for rheumatologic manifestations (migrant arthritis and tenosynovitis), without psoriasis or family history of psoriasis, gastroenteric manifestations, or recent genitourinary infections. The instrumental and laboratory tests have suggested a diagnosis of undifferentiated seronegative HLA-B27-positive spondyloarthritis with predominantly peripher...

متن کاملSpotlight in Clinical Response Sustained Remission of Multicentric Castleman Disease in Children Treated with Tocilizumab, an Anti-Interleukin-6 Receptor Antibody
Multicentric Castleman Disease (MCD) is an idiopathic lymphoproliferative disorder, reported exceptionally in children and generally believed to be an autoinflammatory disease resulting in an increase of interleukin-6 secretion. Previous studies in adult patients suggested a beneficial role of the anti-interleukin-6 receptor antibody tocilizumab on the clinical and biologic disease manifestatio...

Introduction Because most children with oligoarticular juvenile idiopathic arthritis (o-JIA) were mildly affected (Steinblocker functional class I, c.a.85%), o-JIA is tend to be thought of as a mild subtype of JIA. However, 6% of them were unable to participate in a full school program 5 years after diagnosis, and 0.5% of children with o-JIA progressed to class III or IV, severe to terminal sta...
